Ontario Health Teams

Ontario Health Teams are responsible for organizing and delivering health services in local communities. Their goal is to provide individuals with more connected and convenient care. These teams consist of various health care providers from both the health and community sectors, who work collaboratively as a unified team. About the Eastern York Region North Durham OHT

Krasman Centre is a proud member of the Eastern York Region North Durham (EYRND) OHT.

The Eastern York Region North Durham Ontario Health Team (EYRND OHT) serves the Township of Uxbridge, the City of Markham, the Town of Whitchurch-Stouffville and the community of Thornhill, with the mandate to facilitate and coordinate connected, seamless, people-centred care 24/7 for local residents. The EYRND OHT focuses on the development of integrated services for mental health and
addiction, people living with dementia and their caregivers, unattached patients, frail/complex older adults, and patients with chronic conditions.

About the Northern York South Simcoe OHT

Krasman Centre is a proud member of the Northern York South Simcoe OHT.
The Northern York South Simcoe Ontario Health Team consists of a group of healthcare providers and organizations who work together to improve both access to care for patients, and the quality of that care. The geographical area which we serve includes the Town of Aurora, Town of Newmarket, Town of East Gwilimbury, Town of Georgina, and the Town of Bradford. Our goal is to improve the health of the
broader population over time and increase collaboration which will lead to improved experiences for both patients and providers.
